Omni Medical Partners LLC works exclusively with one trusted pharmacy partner to fulfill your medication orders. All shipping timelines, delivery methods, and order processing procedures are determined by the pharmacy.
We do not control or influence the pharmacy’s shipping schedule and cannot expedite or modify delivery timelines once your order has been submitted.

Shipping and Delivery
Omni Medical Partners LLC is not liable for delays, lost shipments, or other issues that may occur during the shipping process. These matters fall under the responsibility of the pharmacy and the shipping carrier (e.g., UPS or FedEx).
If any issues arise with your shipment, please contact the pharmacy directly. We are happy to assist you in initiating communication with them if necessary.

Address Confirmation
Please ensure your shipping address is correct and complete before submitting your order. We cannot replace or refund medications that are not delivered due to inaccurate or incomplete address information provided during your order.
Scheduling Policy

At Omni Medical Partners LLC, we strive to provide a seamless experience for every patient. Please review our appointment and refund policies carefully to avoid any disruptions in care.

Rescheduling & Cancellations
We kindly ask that you cancel or reschedule your appointment at least 24 hours in advance of your scheduled time. If you need to reschedule or cancel, you may do so through the booking link (subject to availability).
Refunds

Cancellations made 24 hours or more in advance are eligible for a refund minus a 5% card processing fee.

If we must cancel an appointment on our end, you will receive a full refund or credit for the full amount, whichever you prefer.
All refund requests must be submitted via email. Refunds are not issued automatically.

Missed Appointments ("No-Shows")
If you do not attend your scheduled virtual visit, it will be marked as a No-Show. We will attempt to contact you up to three times at the phone number or email you provided at booking.
Please ensure your contact information is accurate prior to your appointment. If you need to update your information, email us at least 2 hours before your scheduled time.

Incomplete Forms
Failure to complete all required intake forms prior to your appointment may result in the visit being canceled at the discretion of your provider. You will receive a confirmation email after form submission—please double-check that everything is completed in advance.
Telehealth Informed Consent

Section 1: Understanding the Scope of Services
Omni Medical Partners LLC is a medical practice based in Maryland that offers weight loss care, including prescription medications and clinical support, via a telehealth platform. We provide consultations through secure virtual visits with licensed healthcare providers. Our focus includes evidence-based treatment plans involving lifestyle guidance and, when appropriate, prescription medications such as GLP-1 agonists and related therapies.
Omni Medical Partners LLC is not a pharmacy, and we do not dispense medications directly. We collaborate with a single licensed compounding pharmacy to fulfill prescriptions. Once prescribed, your medication will be shipped to you directly by the pharmacy. Omni Medical Partners LLC does not control the pharmacy’s processing times or shipping methods.
By proceeding with our services, you acknowledge and understand that:

You are entering into a physician-patient relationship solely with Omni Medical Partners LLC and its licensed providers.

Your treatment is conducted exclusively through telehealth, and we do not offer in-person care at this time.

Our services are limited to weight loss and associated health conditions (such as prediabetes, insulin resistance, and metabolic syndrome) and are not a replacement for your primary care provider.

We do not offer emergency care. If you are experiencing a medical emergency, call 911 or go to the nearest emergency room immediately.​​
A.Your Responsibilities
You agree to provide accurate, complete, and up-to-date information during your intake and medical consultations. This includes your medical history, medications, allergies, and any relevant lab work or past diagnoses.
Providing false, misleading, or incomplete information may impact the quality of your care and could result in denial of treatment or unintended health risks. Omni Medical Partners LLC is not responsible for adverse outcomes resulting from inaccurate or omitted information you provide.​
B.Limitations of Telehealth
Telemedicine has unique benefits, including convenience, accessibility, and continuity of care. However, there are limitations, including:

Lack of physical examinations

Potential technical issues with audio/video

Limited ability to respond to complex or urgent medical needs
By using our services, you consent to receive care via telehealth and understand its limitations.​
C.Consent to Treatment
By engaging with Omni Medical Partners LLC, you consent to:

Receiving telehealth services from a licensed Maryland provider

The use of secure electronic communication for consultations, treatment plans, and follow-up care

The potential prescription of weight loss or metabolic medications when clinically appropriate
You also understand that the decision to prescribe any medication is at the discretion of your healthcare provider, based on your individual medical history, lab results, and current health status.

Section 2: Eligibility to Use Our Services
Omni Medical Partners LLC currently provides telehealth weight loss services exclusively to patients located in the State of Maryland. To receive care, you must be physically located in Maryland at the time of your consultation and must be at least 18 years of age.
By using our services, you affirm that:

You are capable of entering into a legally binding agreement.

You are not barred from receiving telemedicine services under any applicable federal, state, or local laws.

You agree to comply with all relevant regulations, including those related to telehealth and prescription medication.
We reserve the right to modify, suspend, or discontinue any aspect of our services without prior notice. This may include changes to available features, service scope, or operational policies. We also retain the right to establish usage limits or storage requirements as necessary.

Section 3: Account Registration and Medical Evaluation
To access telehealth services through Omni Medical Partners LLC, you must first create an account and complete the necessary intake forms, which include providing your medical history, current medications, allergies, and other relevant health details.
Once your intake is complete, a licensed Maryland provider will:

Review your health information

Conduct a virtual consultation (via secure video or audio platform)

Determine the appropriateness of any medical treatments, including prescription medications such as GLP-1 receptor agonists
The decision to prescribe any medication is made solely at the provider’s clinical discretion. Controlled substances (DEA Schedule medications) will not be prescribed under any circumstances. If your provider does not believe treatment is medically appropriate, no prescription will be issued.
We partner with a licensed compounding pharmacy to dispense medications if prescribed. If you prefer to use a different pharmacy, we can send your prescription elsewhere upon request, and you will be responsible for the cost and coordination of filling the prescription.
Please note: We do not accept returns of prescription products once dispensed, in compliance with pharmacy regulations.

Section 4: Payments and Billing
At Omni Medical Partners LLC, we operate on a direct-pay model. We do not accept insurance or submit claims to any federal or state healthcare programs, including Medicare or Medicaid. All services—including clinical care, virtual visits, and medications—are self-pay only. You are solely responsible for all associated costs.
Please carefully review the following terms regarding payments, billing, cancellations, and refunds.
A. Membership Fees
A non-refundable membership fee is required to access services. This fee covers:

Care coordination and administrative processing

Access to licensed providers

Platform and technology maintenance
Your membership remains active as long as the fee is current. Membership does not guarantee access to specific services unless medically appropriate and initiated by you.
B. Virtual Visit Payments
All medical consultations are conducted via secure telemedicine platforms (not telephone consults).
Once payment is submitted for an initial or follow-up virtual visit, it is non-refundable, as clinical processing—including chart review and intake—begins immediately.
You will only be charged for future visits or refill assessments if you actively request them.
C. Medication Orders
Medication costs are separate from consultation fees and are determined by your treatment plan. These orders are processed through our pharmacy partners (e.g., Empower Pharmacy), and:

Once a prescription is submitted, it cannot be canceled or refunded.

Omni Medical Partners LLC is not responsible for pharmacy-related issues such as delays, product quality, or shipping errors. These must be addressed directly with the pharmacy.
Costs may vary by dosage, formulation, and changes to your prescription. You authorize billing at the time of prescription approval and fulfillment.
D. Cancellations

Virtual visits are non-refundable once submitted, as clinical review begins immediately.

If a telephone visit (when offered) is canceled in advance, you may be eligible for a partial refund in accordance with our scheduling terms at the time of booking.

Membership cancellations are effective the following billing cycle but are not retroactively refundable.
E. Payment Processing
You must enter valid payment information at the time of booking or service initiation. By doing so, you authorize Omni Medical Partners LLC and our third-party processor to charge your account for agreed-upon services.
F. Payment Errors and Refund Requests

If you make a duplicate payment or submit a transaction in error, email us promptly.

If no services or medication orders have been initiated, we will review your refund request.

Approved refunds will have payment processor fees deducted, as these fees are non-recoverable by us.
Please verify all charges before completing payment.
G. Chargebacks and Invalid Disputes
If you initiate a chargeback without valid cause, the following terms apply:

A $50 administrative fee will be added to the balance due.

Your account may be suspended, and services paused until payment is resolved.

You have 30 days from notice to resolve the matter before your account is referred to collections.

Notice will be sent to your registered email address.
Multiple invalid disputes may result in permanent termination of access to our services.
H. Order Modifications
Changes to a prescription order after payment is submitted are not guaranteed. In rare cases, we may accommodate modifications if:

The pharmacy has not yet started processing

The change is clinically appropriate and approved by a provider
This is solely at the discretion of Omni Medical Partners LLC.

You retain ownership of any content you upload or submit to Omni. However, you grant us a non-exclusive license to use, host, and process your content for the purpose of delivering our services.
You also agree that any feedback or suggestions you provide may be used by Omni without obligation or compensation.

Upon termination, certain provisions of these Terms will remain in effect, including those regarding intellectual property, indemnity, and limitations of liability.
In accordance with applicable law, we will retain your medical records for at least five (5) years, or longer as required.

Section 11. Disclaimers, Liability Limitations, and Indemnification
A. Services Provided “As Is”
Omni Medical Partners LLC provides the services “as is” and “as available” without warranties of any kind, either express or implied. We do not guarantee:

That the services will meet your specific needs.

The accuracy of content provided by physicians, labs, or pharmacies.

Continuous, uninterrupted, or error-free service.
You use our services at your own risk.
If you are a California resident, you waive the protections of California Civil Code Section 1542, which states:
“A general release does not extend to claims which the creditor does not know or suspect to exist in their favor at the time of executing the release…”
B. Third-Party Services and Links
Our platform may include links or access to third-party content and services, such as compounding pharmacies or labs. Omni is not responsible for:

The accuracy or reliability of third-party content.

Any damages or issues arising from third-party services.
C. Limitation of Liability
To the fullest extent permitted by law, Omni’s total liability to you for any claim related to these services is limited to the greater of $100 or the total amount you paid for services in the past six (6) months.
We are not liable for indirect, incidental, punitive, or consequential damages, including loss of data, profits, or goodwill.
D. Indemnification
You agree to indemnify, defend, and hold harmless Omni and its affiliates, officers, employees, and agents from any claims, damages, or liabilities arising out of:

Section 12. Governing Law and Dispute Resolution
These Terms are governed by the laws of the State of Maryland, without regard to conflict of law principles.

Any dispute or claim arising from these Terms or your use of the services will be resolved through binding arbitration under the rules of the American Arbitration Association (AAA), rather than in court.

The arbitration will take place in Baltimore County, Maryland, unless otherwise agreed in writing.

Each party will bear its own arbitration costs, but the arbitrator may award costs and attorneys’ fees in accordance with applicable law.
You and Omni waive any right to a jury trial and agree that any arbitration will be conducted only on an individual basis, not in a class or representative action.
If a claim qualifies for small claims court, either party may bring an individual claim in small claims court instead of arbitration.

Weight Loss Bill of Rights
The Weight-Loss Consumer Bill of Rights shall consist of the following provisions:

(A) WARNING: RAPID WEIGHT LOSS MAY CAUSE SERIOUS HEALTH PROBLEMS. RAPID WEIGHT LOSS IS WEIGHT LOSS OF MORE THAN 11/2 POUNDS TO 2 POUNDS PER WEEK OR WEIGHT LOSS OF MORE THAN 1 PERCENT OF BODY WEIGHT PER WEEK AFTER THE SECOND WEEK OF PARTICIPATION IN A WEIGHT-LOSS PROGRAM.

(C) ONLY PERMANENT LIFESTYLE CHANGES, SUCH AS MAKING HEALTHFUL FOOD CHOICES AND INCREASING PHYSICAL ACTIVITY, PROMOTE LONG-TERM WEIGHT LOSS.

(D) QUALIFICATIONS OF THIS PROVIDER ARE AVAILABLE UPON REQUEST.
